The Company accepts the following notification methods for personal information disclosure, correction, content erase, stop of usage, complaints, inquiries and request to stop providing them to a third party.
However, we will not disclose, correct, erase, add, stop using, stop providing to third parties in case determined by law, or requested not by a method described below.

For more information on accepted procedures, please note that for security reasons some information would be requested to make sure of the customer identity (or his/her representative). In addition, depending on the customer request, there might be the need of submitting a written application prescribed by the Company, as described in the following.
For personal information disclosure, correction, content erase, stop of usage, complaints, inquiries and request to stop providing them to a third party, please perform your application by the following method.
1.Please download and print the "Personal Information Contact application" form (PDF file).
2.Please fill the form in and send it by registered mail only to the following address along with matching identification documents’s copies.

※The Company will respond without delay to our customers’s requests. Please note it may take some time to approve the application and notify the result. The notification will be sent back to our customer by registered mail only, together with the received identification documents’s copies. It should be noted that, due to safety and security reasons involving both the application form and the enclosed documents, only registered mail will be accepted.
When enclosing and sending to the Company important documents, it will be appreciated if the customer could mark the envelope with "Request Form Enclosed” wrote in red.

If you are a representative person delegated by the customer requesting the personal information disclosure, or a legal representative for a minors, in addition to the above necessary documents described in (4), please enclose also the following documents (A or B).

1,500 JPY fee for each application.
Please enclose together with the application documents a flat-rate postal order of 1,500 JPY (issued at post offices).
※ Please notice that shipping cost should be covered by the client.
※ Please note that if the fee would not be enough, or if the fee has not been enclosed, and no payment adjustment within the prescribed period after the notification, the entire application should be considered invalid.
A written notice will be addressed to the customer and the reported billing address by mail.
Personal information obtained in accordance with customer’s request for disclosure, would be used only on the necessary extent to complete the disclosure procedure.
There is the possibility that the submitted application will result invalid. Please refer to the cases below, and please note that the application fee will not be reimbursed.
・If the customer cannot be identified. For example if the identification address indicated in the application and the address listed on the request does not match
・If the customers representative cannot be identified
・If the subject of request for disclosure does not correspond to anyone for which the Company retains personal data
・If there is a risk of damaging the rights and interests of third party person life, body or property
・If there is a risk to seriously impede the proper implementation of the Company’s business
・ If there is a violation in the law or regulations
